The Madwaleni HBC programme seeks to enable
chronically ill or palliative, clinically stable patients to be cared for holistically
within their home environment by assisting and empowering the patient together
with his/her family members and responsible community health worker.This programme allows professional staff at the hospital and clinics to refer patients for home based care knowing that they will continue to be supported and followed up by the team. Patients benefiting from this service include:
- patients who require continued rehabilitation
- chronically ill patients including patients with HIV, TB, hypertension, diabetes
- palliative care patients including patients that have cancer or suffered from strokes
The home based care team is made up of the home based care co-ordinator, a doctor, a physiotherapist, an occupational therapist, home based carers, a nurse at each clinic and the community health workers based at each of the 8 feeder clinics.
